Which is the primary cause of death in fires?

hope that completely happens before the fire sets in..what an extremely horrible way to go out!

Have seen a few deaths on the drama programs on TV, now I know that they aren't real, but it is dramatic. and the only way to get out of a burning building is to have a planned escape route, so you know which way to turn and get out.
