I suspect the actual phrase that will be used to communicate to the PM that the Queen has passed away will be "the Queen has passed away", which is only one word longer but doesn't need to he written down in a code book

AQET, depends on the manner in which she dies. If it's publicly then the secret service, or whatever the British equivalent is, would say over the ear pieces "London Bridge is down". Also if she dies while the PM is in a meeting then the aide that tells him would say the code phrase so that whomever the PM is meeting with wouldn't be automatically alerted to the delicate state the country is now in. It would be the same in any country if their main leader dies.
