Unfortunately, we are infallible human beings who make mistakes. Since this was not a deliberate act, it's hard to make extreme judgements about what happened. Yes, it is extremely sad that so many people lost their lives on March 27, 1977. Hopefully, some major changes have been made in Aviation, over the years, so that this tragedy will not happen today.

Player #62086564, Sadly, this was deliberate. The pilot of the KLM flt ignored protocol and made the decision to take off without clearance from the tower. He was stopped once by his first officer and engineer, but the second effort, he insisted that "We go." Hence the omission of the "hierarchy" in the cockpit.

The pilot from Pan Am and the air traffic controller tried to tell the KLM flight that Pan Am was still on the runway. They spoke at the same time and canceled each other out. the KLM flight didn't hear them.

This really reminded me of the 9/11 attacks. I really thought that would be the answer - I don’t know if it caused the most deaths or casualties or injured people, but it was certainly the most horrifying. I know most people waited with baited breath to see if there were any survivors. I myself had a family member get out just in time. My best friend also luckily left the building just one hour before the fatal plane crash. It was the scariest day of my life.
