What does it mean when the umpire rotates their arm in the air or in a 'windmill' motion in cricket?

Test cricket is the default. I don't know the signal referred to. But I thought they might have meant four runs. I cannot remember seeing a windmill signal. I don't watch 20/20, only tests and the occasional one day.
