To the best of my knowledge, it's always the bishop. Maybe independent priests, but normally the various churches send a priest to the cathedral for the annual Chrism Mass, during which the oil is blessed by the bishop by breathing upon it. It is reminiscent of Christ breathing upon the Apostles to transmit the Holy Spirit.
