Oh yeah, the circus in the United States is dead thanks to PETA. Once the animals were gone, interest died and Ringling Brothers closed up shop.

Tina, you are correct. His character was a medical doctor who was hiding from the law because of a mistake he once made. His identity is revealed after the climactic train wreck, where his medical expertise comes in handy at saving lives.

I believe that the back story to Jimmy Stewart's character is that he wares the clown make up all the time because he is a wanted criminal. He believes no one will recognize him and that's why he is with the circus.
